Options are maxBytes, timeoutMs (1 to 300,000 ms, default 5,000), and
trustedProxies. Each request is captured under its own fresh timeoutMs deadline.
A context is refused with invalid_options: one Context is one absolute deadline,
and the registration outlives it.
